Foster/Volunteer Application

Thank you for your interest in becoming a Cats at the Studios, Inc. foster parent or volunteer!  Please take a moment to read the policies and requirements below, then fill out the form to apply.

FOSTER PARENT/VOLUNTEER AGREEMENT

As a Foster Parent/Volunteer, I understand that CATS AT THE STUDIOS, INC. (“CATS INC.”) takes care of all medical expenses incurred during my fostering of cats/kittens.  CATS, INC. will make available veterinary care for all foster animals through designated “Partner Veterinarians” only.  Medical expenses must be pre-approved and performed by one of CATS INC. rescue vets. If veterinary care and services are not pre-approved, expenses incurred are the financial responsibility of the Foster Parent/Volunteer. Foster Parent/Volunteer must be available to transport foster animals to the vet as needed, will agree to administer proper medications as prescribed, and will abide by any recommendations made by the animals’ veterinarian and/or CATS INC.

As a Foster Parent/Volunteer, I agree to spay or neuter all cats and kittens with a pre-approved vet and make sure the cat or kitten has vaccinations by a representative of CATS INC. and is tested for FeLV/FIV prior to bringing same to adoption events. I further agree to maintain detailed records of dates, places and certificate of spay/neuter, testing, vaccinations, de-worming and de-fleaing and complete medical history and provide information to prospective adoptive parents and CATS, INC.

Foster Parent/Volunteer will make no decisions or take any actions regarding the animal(s) veterinary care (including euthanasia) or placement (temporary or permanent) without prior approval from CATS, INC.  I understand that CATS, INC. retains ownership of all foster animals until otherwise advised.

Foster Parent/Volunteer agrees to keep the animal indoors at all times.

Foster Parent/Volunteer agrees to notify CATS INC.’s Foster Coordinator immediately of any kind of problem concerning the animal that should arise during foster care.

If the Foster Parent/Volunteer cannot keep the animal, they will notify CATS INC’s Foster Coordinator immediately. Foster Parent agrees to give CATS, INC. as much notice as possible, and understands that while alternative arrangements will be made in a timely manner, space availability varies and time constraints will dictate the amount of time needed to facilitate the animal’s return.

If the Foster Parent/Volunteer is unable to fully socialize and tame their foster animals(s) and they are deemed not adoptable by the Foster Coordinator, the Foster Parent/Volunteer will take responsibility for finding a humane living situation for their foster animal(s).

As the Foster Parent/Volunteer, I am responsible for purchasing and providing the food given to the animal. I understand that I receive foster care animals at my own risk.

I indemnify and hold CATS INC. free and harmless from all liability arising out of any and all claims, demands, losses, damages, action, judgment of every kind and description which may occur to or be suffered by me, members of my household or my personal pets, or any third parties by reason of activities arising out of this Agreement.
